Schedule/Hours:
- Minimum 36-hour/week guarantee with assigned productivity points (if guarantee applicable)
- Weekend coverage may be required
- Occasional travel outside designated territory to maintain caseloads
Qualifications:
- Active California Physical Therapist License
- Current BLS certification
- Valid drivers license, car insurance, and auto registration
- Up-to-date immunizations: TB (annual), MMR, Varicella, HepB, Tdap (within 10 years), Influenza (annual)
- Recent drug screen and physical
- Strong documentation and organizational skills
Responsibilities:
- Deliver high-quality therapy services in hospital and home health settings across assigned counties
- Complete OASIS, evaluations, recertifications, discharges, and routine visits per established standards
- Maintain thorough, timely documentation and coordinate care with patients, families, and providers
- Adapt to varying caseloads and geographic assignments to ensure optimal patient outcomes
- Provide care to Covid+ or Covid PUI patients, as needed
