Schedule/Hours:

  • Productivity typically set at a minimum of 27 points per week (points equate to types of visits and documentation)
  • 36-hour workweek guarantee applies if productivity and flexibility requirements are met
  • Weekend coverage and occasional travel outside the core area may be required

Preferred Qualifications:

  • Current California Physical Therapist license (PT)
  • BLS certification
  • Valid drivers license, auto insurance, and auto registration
  • Up-to-date health records, including TB (annual), MMR, Varicella, HepB, Tdap (10 years), and annual flu vaccination
  • Willingness to see Covid+ and Covid PUI patients
  • Strong organization and adaptability skills

Primary Responsibilities:

  • Provide comprehensive physical therapy care to patients in their homes across multiple communities
  • Complete evaluations, OASIS documentation, recertifications, and discharge planning per industry standards
  • Maintain accurate, timely electronic documentation and coordinate care with interdisciplinary teams
  • Manage a caseload requiring both clinical expertise and autonomy in scheduling
  • Effectively communicate with patients, families, and healthcare providers
