Qualifications & Desired Experience:
- Bachelors degree in Physical Therapy (Masters or Doctorate preferred)
- Active PT license in MA
- Prior experience in long-term care or a skilled nursing facility preferred
- Basic Life Support (BLS) certification preferred
- Proficient communication, interpersonal, and documentation skills
- Ability to lift up to 50 pounds and perform physical demands of clinical work
- Strong decision-making, time management, and collaborative abilities
Core Responsibilities:
- Evaluate resident needs and create individualized therapy plans
- Collaborate on multidisciplinary care teams and interdisciplinary meetings
- Deliver direct therapeutic interventions and monitor progress
- Educate residents and caregivers on assistive devices and rehabilitation routines
- Maintain detailed, regulatory-compliant documentation in the electronic medical record
- Mentor new team members and supervise assistants as appropriate
- Contribute to program and service development within rehabilitation
