Qualifications & Desired Experience:

  • Bachelors degree in Physical Therapy (Masters or Doctorate preferred)
  • Active PT license in MA
  • Prior experience in long-term care or a skilled nursing facility preferred
  • Basic Life Support (BLS) certification preferred
  • Proficient communication, interpersonal, and documentation skills
  • Ability to lift up to 50 pounds and perform physical demands of clinical work
  • Strong decision-making, time management, and collaborative abilities

Core Responsibilities:

  • Evaluate resident needs and create individualized therapy plans
  • Collaborate on multidisciplinary care teams and interdisciplinary meetings
  • Deliver direct therapeutic interventions and monitor progress
  • Educate residents and caregivers on assistive devices and rehabilitation routines
  • Maintain detailed, regulatory-compliant documentation in the electronic medical record
  • Mentor new team members and supervise assistants as appropriate
  • Contribute to program and service development within rehabilitation
