Qualifications:

  • Bachelors degree in Physical Therapy required; advanced degree (Masters/Doctorate) preferred
  • Active, valid Physical Therapist license in the applicable state
  • Basic Life Support (BLS) certification preferred
  • Previous experience in long-term care or skilled nursing facilities a plus
  • Strong verbal, written, and interpersonal skills
  • Proficient in Microsoft Office and Electronic Medical Records
  • Compassionate, resident-centered approach to care

Key Responsibilities:

  • Perform evaluations and therapeutic procedures following physician orders
  • Develop and document treatment plans with measurable goals
  • Collaborate and communicate with residents, families, and healthcare team members
  • Provide education on therapy techniques and equipment
  • Supervise assistant staff, mentor new team members, and participate in department initiatives
  • Maintain compliance with Medicare/Medicaid and other regulatory standards
  • Assist with cleanliness, infection control, and equipment maintenance
