Job Description

Deliver comprehensive care as a licensed Physical Therapist, providing outstanding therapy services within a hospital setting. This contract position offers a rewarding opportunity to serve patients across counties, focusing on detailed evaluations and evidence-based treatments as part of a collaborative healthcare team.

Schedule/Hours:

  • Monday to Friday, 8:00am to 5:00pm
  • No on-call required

Key Qualifications:

  • Active Georgia Physical Therapist license or compact license eligibility
  • Minimum 2 years of experience in home health and with HomeCare Homebase preferred
  • Strong clinical assessment, planning, and documentation skills
  • Ability to travel within Lowndes, Echols, Brooks, Cook, Berrien, and Lanier counties (mileage reimbursement provided)
  • Knowledgeable in federal, state, and professional compliance standards

Primary Responsibilities:

  • Complete thorough physical therapy evaluations based on physician orders/referrals
  • Develop and implement individualized treatment plans with measurable goals
  • Regularly reassess patient progress, adjusting care plans as needed
  • Collaborate and communicate with patients, families, physicians, and interdisciplinary teams
  • Assist patients in acquiring adaptive equipment to enhance rehabilitation
  • Accurately document evaluations, treatments, and billing per required protocols
  • Uphold company compliance, ethics policies, and HIPAA standards
  • Supervise Physical Therapist Assistants and Aides in accordance with state practice guidelines
  • Participate in professional development, training sessions, and internal meetings
