Key Qualifications:

• Minimum of 2 years of professional experience in hospital-based therapy, preferably within home health settings
• Current, active Physical Therapy license for the state of California
• Valid Basic Life Support (BLS) certification
• Demonstrated expertise assessing, planning, and delivering individualized therapy care in the home setting
• Strong organizational, communication, and documentation abilities

Primary Responsibilities:

• Conduct initial and ongoing patient assessments within the home environment, creating tailored therapy plans to support patient goals
• Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care
• Educate patients and families on home exercise programs, safety protocols, and progress toward recovery
• Continuously monitor, evaluate, and adapt therapeutic interventions based on patient response
• Maintain accurate, timely records and comply with all regulatory and professional standards
