Key Qualifications:
- Minimum of 2 years of professional experience in hospital-based therapy, preferably within home health settings
- Current, active Physical Therapy license for the state of California
- Valid Basic Life Support (BLS) certification
- Demonstrated expertise assessing, planning, and delivering individualized therapy care in the home setting
- Strong organizational, communication, and documentation abilities
Primary Responsibilities:
- Conduct initial and ongoing patient assessments within the home environment, creating tailored therapy plans to support patient goals
- Collaborate with interdisciplinary healthcare teams to coordinate and optimize patient care
- Educate patients and families on home exercise programs, safety protocols, and progress toward recovery
- Continuously monitor, evaluate, and adapt therapeutic interventions based on patient response
- Maintain accurate, timely records and comply with all regulatory and professional standards
