Schedule/Hours:

• Flexible scheduling with periodic office meetings and home health visits

Responsibilities:

• Perform patient evaluations and provide therapy services in adherence with physicians' orders and state guidelines
• Develop, update, and manage individualized treatment plans
• Supervise and evaluate Physical Therapy Assistants and Home Health Aides as assigned
• Observe, record, and report patient progress and conditions to the clinical team
• Educate, advise, and consult with patients, families, and staff on therapy protocols and home environments
• Participate in professional development, in-service education, and maintain accurate documentation
• Coordinate care consistently with state-specific regulations and clinical best practices

Desired Qualifications:

• Graduation from an accredited physical therapy program
• Current, unencumbered physical therapy license in the applicable state
• 2-3 years of clinical physical therapy experience preferred
• Current CPR certification
• Strong interpersonal and communication skills
• Proficient with basic computer operations and online clinical documentation
• Reliable transportation, valid drivers license, and required insurance
