• Sunday Thursday
• Tuesday Saturday

Qualifications:

• Active California Physical Therapist license required
• Previous experience in a skilled nursing facility preferred; strong mentorship available for candidates with 01 year of experience
• Excellent clinical documentation and time management skills
• Flexible, collaborative approach to patient care
• Willingness to float to a sister facility located within walking distance, if needed

Key Responsibilities:

• Perform comprehensive evaluations and design individualized treatment plans to enhance strength, mobility, and functional abilities of patients
• Deliver effective therapy interventions for a diverse caseload within a skilled nursing environment
• Work closely with an interdisciplinary care team to ensure integration of therapy goals into each patients overall care plan
• Accurately and efficiently document all assessments, progress notes, and outcomes using Net Health in accordance with facility standards
