This travel opportunity is designed for those eager to broaden their clinical expertise by working within diverse healthcare teams and experiencing a variety of patient needs and leadership styles. The position offers the chance to collaborate across multiple counties, enriching your resume and offering valuable exposure to different rehabilitation approaches and documentation systems.

Schedule/Hours:

  • Monday to Friday, 8:00am 5:00pm

Qualifications:

  • Active Physical Therapist license
  • Previous home health experience required
  • Familiarity with Homecare Homebase (HCHB) preferred
  • Strong evaluation, assessment, and treatment planning skills
  • Ability to supervise PT Assistants and Aides
  • Valid drivers license for travel between patient homes (mileage reimbursed at $0.43/mile)

Key Responsibilities:

  • Conduct comprehensive patient assessments and develop individualized treatment plans
  • Set and regularly reassess measurable goals, modifying interventions as needed
  • Maintain accurate, timely therapy documentation in line with regulatory standards
  • Effectively communicate patient progress with care teams, families, and referring physicians
  • Recommend adaptive equipment and integrate into therapy as appropriate
  • Ensure compliance with corporate standards, HIPAA, and professional best practices
  • Participate in staff meetings and continuing education to keep clinical skills sharp
