Schedule/Hours:
- Monday to Friday, 8:00am 5:00pm
Qualifications:
- Active Physical Therapist license
- Previous home health experience required
- Familiarity with Homecare Homebase (HCHB) preferred
- Strong evaluation, assessment, and treatment planning skills
- Ability to supervise PT Assistants and Aides
- Valid drivers license for travel between patient homes (mileage reimbursed at $0.43/mile)
Key Responsibilities:
- Conduct comprehensive patient assessments and develop individualized treatment plans
- Set and regularly reassess measurable goals, modifying interventions as needed
- Maintain accurate, timely therapy documentation in line with regulatory standards
- Effectively communicate patient progress with care teams, families, and referring physicians
- Recommend adaptive equipment and integrate into therapy as appropriate
- Ensure compliance with corporate standards, HIPAA, and professional best practices
- Participate in staff meetings and continuing education to keep clinical skills sharp
