Schedule/Hours:

• Travel position with flexible scheduling
• Periodic office meetings and home visits as part of your responsibilities

Key Responsibilities:

• Perform thorough patient evaluations and implement therapeutic services as per physician orders
• Supervise and mentor Physical Therapy Assistants and Home Health Aides
• Assist physicians in evaluating patient function and updating care plans
• Document and report patient conditions in real time, maintaining up-to-date health records
• Consult and educate patients' families and agency staff on therapy programs
• Evaluate home environments, providing recommendations to optimize therapy outcomes
• Ensure compliance with relevant state regulations and standards for therapy services
• Regularly attend in-service educational programs to stay at the forefront of best practices

Desired Qualifications:

• Graduation from an accredited physical therapy program
• Current, unencumbered state PT license as required
• Minimum 2-3 years of hands-on experience in physical therapy (preferred)
• Effective oral and written communication skills
• CPR certification
• Proficient with general computer operations and electronic documentation
• Reliable transportation, valid drivers license, and liability insurance
