Key Qualifications:
- Valid and current Physical Therapist (PT) license
- Minimum of one year of recent experience in a hospital or acute care setting preferred
- Strong assessment, documentation, and communication skills
- Ability to work collaboratively with physicians, nurses, and therapists
- Current BLS certification (American Heart Association) required
- Excellent patient care and organizational abilities
Responsibilities:
- Conduct thorough assessments and develop individualized treatment plans for a diverse patient population
- Deliver evidence-based therapeutic interventions to promote strength, mobility, and functional recovery
- Document evaluations, progress notes, and discharge summaries accurately and promptly
- Educate patients and families on mobility strategies, home exercise regimens, and safe discharge planning
- Collaborate closely with other healthcare professionals to ensure continuity and quality of care
- Support patients throughout their rehabilitation journey, adapting plans as their needs evolve
