Schedule/Hours:
- Weekday and weekend coverage required as needed for patient caseload
- Assignments may include travel within San Jose, Santa Clara, Sunnyvale, Cupertino, Milpitas, and surrounding communities
Qualifications:
- Current California Physical Therapy license (PT)
- Valid BLS certification
- Drivers license, auto insurance, and vehicle registration
- Recent TB screening, MMR, Varicella, HepB, Tdap (within last 10 years), Flu vaccination, drug screen, and physical exam
Responsibilities:
- Provide comprehensive physical therapy evaluation, treatment, and discharge planning for home health patients
- Accurately document visits, communications, and care plans
- Participate in OASIS assessments (SOC, Re-certification, RoC)
- Adapt quickly to new environments and travel as required to meet caseload demand, including occasional weekends
- Collaborate with physicians, caregivers, and the healthcare team to ensure the highest care standards
