Job Description

Seeking a passionate physical therapist to deliver quality rehabilitation services within a hospital setting. In this role, you will provide care to patients in both the acute inpatient unit and the long-term care facility. Collaborate with a dedicated interdisciplinary team to support patient recovery and enhance functional abilities. This contract opportunity allows you to work in a dynamic environment, making a meaningful difference for patients from Macon, Jackson, and the surrounding counties.

Schedule:

  • Contract position
  • Work in both Acute Care and Long-Term Care settings

Qualifications:

  • Valid Physical Therapy license
  • Experience with hospital-based acute care and/or long-term care preferred
  • Strong assessment, documentation, and communication skills
  • Compassionate approach to patient care and rehabilitation
  • Ability to collaborate effectively with patients, families, and the healthcare team

Key Responsibilities:

  • Evaluate and treat patients within the acute inpatient and long-term care units
  • Develop individualized treatment plans to maximize patient mobility and functional independence
  • Educate patients, family members, and caregivers on therapy goals and exercises
  • Accurately document all care provided in the electronic health record system
  • Participate in team conferences, discharge planning, and ongoing interdisciplinary collaboration
