Key Qualifications:

• Bachelors degree in Physical Therapy required; Masters or Doctorate preferred
• Active Physical Therapist license in Massachusetts (or eligibility)
• Skilled Nursing Facility/long-term care experience required
• BLS certification preferred
• Strong written, verbal, and interpersonal communication skills
• Ability to work both independently and collaboratively
• Proficient in Microsoft Office and electronic medical records
• Must be physically able to lift/carry up to 50 lbs (team lift for above)

Primary Responsibilities:

• Assess, plan, and effectively deliver individualized therapy services under physician orders
• Establish and revise therapeutic plans and functional goals tailored to each resident
• Educate residents and caregivers on therapy plans and assistive devices
• Collaborate with interdisciplinary teams and refer/responsibly supervise PTA staff
• Maintain accurate documentation in EMR, adhering to Medicare/Medicaid and other regulations
• Contribute to program development, staff education, and mentorship of new team members
• Ensure a clean, safe department and equipment in compliance with infection control standards
