Key Qualifications:
- Bachelors degree in Physical Therapy required; Masters or Doctorate preferred
- Active Physical Therapist license in Massachusetts (or eligibility)
- Skilled Nursing Facility/long-term care experience required
- BLS certification preferred
- Strong written, verbal, and interpersonal communication skills
- Ability to work both independently and collaboratively
- Proficient in Microsoft Office and electronic medical records
- Must be physically able to lift/carry up to 50 lbs (team lift for above)
Primary Responsibilities:
- Assess, plan, and effectively deliver individualized therapy services under physician orders
- Establish and revise therapeutic plans and functional goals tailored to each resident
- Educate residents and caregivers on therapy plans and assistive devices
- Collaborate with interdisciplinary teams and refer/responsibly supervise PTA staff
- Maintain accurate documentation in EMR, adhering to Medicare/Medicaid and other regulations
- Contribute to program development, staff education, and mentorship of new team members
- Ensure a clean, safe department and equipment in compliance with infection control standards
