Schedule:
- Full-time, five 8-hour shifts per week
- Includes every other Saturday
- Weekend, holiday, floating, and on-call requirements as needed

Qualifications:
- Graduate of an APTA-accredited school of Physical Therapy
- Active PT license
- Current Basic Life Support (BLS) certification
Key Responsibilities:
- Evaluate patient functional abilities and limitations for home health care
- Develop and implement individualized treatment plans considering patients physical, cultural, social, and developmental needs
- Document care according to professional and regulatory standards
- Communicate effectively within the interdisciplinary team
- Demonstrate flexibility in response to caseload and schedule changes
- Supervise and delegate tasks appropriately to ensure safe, effective care
- Participate in department meetings and professional development
