Schedule:

  • Full-time, five 8-hour shifts per week
  • Includes every other Saturday
  • Weekend, holiday, floating, and on-call requirements as needed

Qualifications:

  • Graduate of an APTA-accredited school of Physical Therapy
  • Active PT license
  • Current Basic Life Support (BLS) certification

Key Responsibilities:

  • Evaluate patient functional abilities and limitations for home health care
  • Develop and implement individualized treatment plans considering patients physical, cultural, social, and developmental needs
  • Document care according to professional and regulatory standards
  • Communicate effectively within the interdisciplinary team
  • Demonstrate flexibility in response to caseload and schedule changes
  • Supervise and delegate tasks appropriately to ensure safe, effective care
  • Participate in department meetings and professional development
