Schedule
- Monday to Friday, 8:00 am 5:00 pm
Qualifications
- Valid Physical Therapist license in AL
- Required: Home Health experience
- Preferred: HCHB experience
- Ability to travel across the specified counties (mileage reimbursement included)
- Strong communication and organizational skills
Responsibilities
- Perform comprehensive PT evaluations and ongoing treatment per physician referrals
- Set measurable goals and develop individualized treatment plans to maximize functional independence
- Re-assess, update, and communicate patient progress and goals to the care team and family
- Collaborate with medical providers and recommend adaptive equipment as needed
- Complete documentation per standard, regulatory, and company requirements
- Supervise PT assistants and aides in direct patient care activities
- Engage in staff development, clinical education, and maintain practice skills
