Desired Qualifications:
- B.S., M.S., or D.P.T. in Physical Therapy
- Current New York State license in Physical Therapy
- Valid BLS certification
- Minimum of one year recent experience in long-term care or home care settings
- Demonstrated proficiency with Oasis documentation
- Strong interpersonal, verbal, and written communication skills
- Ability to provide hands-on patient care, safely lift, and support patients
- Willingness to collaborate across disciplines and with culturally diverse populations
Key Responsibilities:
- Deliver comprehensive physical therapy care and manage patient cases in a home health setting
- Develop, implement, and update individualized plans of care in collaboration with healthcare teams and families
- Document clinical progress, advise patients and caregivers on rehabilitation, and supervise PTA staff
- Support care management and follow agency practices, including compliance with TJC, OSHA, and regulatory standards
- Contribute to ongoing quality improvement and participate in professional education
