Desired Qualifications:

  • B.S., M.S., or D.P.T. in Physical Therapy
  • Current New York State license in Physical Therapy
  • Valid BLS certification
  • Minimum of one year recent experience in long-term care or home care settings
  • Demonstrated proficiency with Oasis documentation
  • Strong interpersonal, verbal, and written communication skills
  • Ability to provide hands-on patient care, safely lift, and support patients
  • Willingness to collaborate across disciplines and with culturally diverse populations

Key Responsibilities:

  • Deliver comprehensive physical therapy care and manage patient cases in a home health setting
  • Develop, implement, and update individualized plans of care in collaboration with healthcare teams and families
  • Document clinical progress, advise patients and caregivers on rehabilitation, and supervise PTA staff
  • Support care management and follow agency practices, including compliance with TJC, OSHA, and regulatory standards
  • Contribute to ongoing quality improvement and participate in professional education
