Schedule/Hours:
- Full-time, 5 x 8-hour shifts
- Includes every other Saturday
- Weekend, holiday, and on-call requirements as needed
- Floating assignments as required
Qualifications:
- Graduate of an APTA-accredited Physical Therapy program
- Active PT license and BLS certification
- Previous hospital therapy experience preferred
- Adaptability for varied caseloads and assignments
Key Responsibilities:
- Evaluate functional abilities and limitations of patients in home health and hospital settings
- Develop and implement individualized treatment plans considering each patients unique physical, social, and cultural needs
- Document care and progress according to regulatory and professional guidelines
- Facilitate smooth patient transitions by communicating vital information within the care team
- Supervise support personnel to ensure safe and effective patient care
- Participate in departmental operations and meetings
