Desired Qualifications
- Graduation from an accredited physical therapy program (APTA/AMA)
- Current, unencumbered state licensure as a Physical Therapist (or eligibility)
- 2-3 years of direct physical therapy experience preferred
- Current CPR certification
- Solid computer skills (navigating files, email, basic software)
Key Responsibilities
- Evaluate patients and implement therapeutic services in accordance with physicians' orders
- Develop, maintain, and adjust individualized treatment plans
- Supervise and evaluate Physical Therapy Assistants and Home Health Aides when assigned
- Document patient status, progress, and report as needed to medical staff
- Educate patients, families, and interdisciplinary staff on therapy programs
- Conduct home visits and participate in periodic office meetings
- Ensure all therapeutic services are performed in compliance with state and federal regulations
- Maintain valid drivers license, reliable transportation, and liability insurance for home visit assignments
