Job Description
Provide skilled physical therapy services in a home health setting, following physician orders to improve patient mobility and function. This contract opportunity is based in Cambridge, MA, offering the chance to support patient wellness through evaluations, treatments, and collaboration with interdisciplinary teams.
Responsibilities include conducting patient evaluations, implementing therapeutic interventions, and updating care plans. You will directly supervise Physical Therapy Assistants and Home Health Aides as assigned. Documentation and communication with attending physicians regarding patient progress are essential components of the role. You will also provide education and guidance to patients, families, and other healthcare personnel, ensuring quality care delivery. Participation in ongoing education and adherence to state-specific regulatory requirements are expected.
Qualifications and Desired Experience:
- Successful completion of an accredited physical therapy program.
- Current, valid physical therapy license for Massachusetts.
- Minimum 2-3 years of physical therapy experience preferred.
- CPR certification is required.
- Proficiency with computer systems for documentation, communication, and training.
- Reliable transportation and ability to conduct home visits.
- Strong interpersonal skills and ability to work independently.
