Schedule/Hours:
- Travel assignment; flexible scheduling options available
Key Qualifications:
- Graduate of an accredited physical therapy program
- Current, unencumbered license to practice physical therapy in the state of assignment
- At least 2-3 years of relevant experience preferred
- CPR certification (current)
- Competence with computers, electronic documentation, and online training
- Valid drivers license, reliable transportation, and required liability insurance
Responsibilities:
- Conduct patient evaluations and provide therapeutic services per physician orders
- Develop, revise, and execute individualized care plans
- Supervise and evaluate assigned Physical Therapy Assistants and Home Health Aides
- Advise families and staff within therapy programs and patient care plans
- Maintain detailed patient records and communicate updates with care teams and physicians
- Participate in office meetings and periodic home visits as necessary
- Stay current with regulatory and agency guidelines and participate in ongoing education
