Job Description
Seeking a skilled physical therapist to deliver specialized therapy services to patients in their homes. This contract role involves supporting individuals with physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. You will work with patients of all ages, collaborating closely with families, physicians, and allied health professionals to deliver customized care and ensure comprehensive treatment.
Schedule/Hours:
- Weekend rotations required once every six weeks
Qualifications:
- Active Physical Therapist professional license in Massachusetts
- Minimum of 1 year of home care experience
- Ability to travel throughout the assigned region to provide in-home patient care
- Strong collaborative skills to work with patients, families, and interdisciplinary teams
- Proficiency in timely and thorough electronic documentation
- Commitment to agency policies, procedures, and standards of practice
Responsibilities:
- Assess patient needs and create individualized therapy care plans
- Deliver physical therapy services in the patient's place of residence
- Monitor progress and adapt treatment strategies as necessary
- Coordinate care with physicians and the interdisciplinary health team to achieve optimal outcomes
- Maintain accurate, timely documentation and clear communication with all involved parties
- Educate patients and families on treatment techniques, progress, and rehabilitation goals
