Job Description

Provide skilled therapeutic services in a home health setting, performing patient evaluations and executing treatment plans according to physicians' orders. This role involves direct supervision and evaluation of Physical Therapy Assistants and Home Health Aides to ensure high-quality patient care. The position requires adherence to state-specific regulations and collaboration with physicians and multidisciplinary teams to optimize patient outcomes.

Key Responsibilities:

  • Conduct comprehensive patient evaluations and develop individualized therapy plans.
  • Administer therapeutic treatments in accordance with physician directives.
  • Supervise and assess Physical Therapy Assistants and Home Health Aides.
  • Document patient progress thoroughly and communicate updates to healthcare providers.
  • Educate patients, families, and agency personnel on therapy programs.
  • Evaluate home environments to recommend modifications that support treatment goals.
  • Participate in continuing education and in-service training programs.
  • Ensure compliance with all applicable state licensing, certification, and regulatory requirements.

Qualifications & Desired Experience:

  • Successful completion of an accredited physical therapy program recognized by national accrediting bodies.
  • Current, unrestricted license to practice physical therapy in the state of assignment.
  • Two to three years of physical therapy experience preferred.
  • Current CPR certification.
  • Proficient with computer applications, including electronic medical records, internet navigation, and online training platforms.
  • Reliable transportation, valid drivers license, and agency-required liability insurance.
