Schedule/Hours:
- Monday Friday
- 8:00am 5:00pm
- No on-call shifts

Desired Qualifications:
- Current West Virginia physical therapy license
- Prior experience in hospital-based physical therapy
- Proficiency with HCHB (Homecare Homebase) electronic health records
- Strong communication and interdisciplinary skills
Key Responsibilities:
- Complete physical therapy evaluations and treatments per physician orders
- Develop and manage patient-centered treatment plans and measurable goals
- Review patient records, assess needs, and adapt interventions as necessary
- Regularly re-assess progress and update care plans for optimal outcomes
- Communicate efficiently with treatment teams, physicians, families, and patients
- Recommend, secure, and integrate adaptive equipment as part of rehabilitation
- Maintain compliance through detailed and timely documentation
- Supervise assistants and aides, ensuring quality patient care
- Participate in staff development and continuing education opportunities
