Schedule/Hours:
- 36-hour weekly guarantee available (must meet productivity expectations)
- Some weekend coverage may be required
- Occasional travel outside of assigned area as needed
Qualifications:
- Active California Physical Therapist license
- BLS certification
- Valid drivers license, auto insurance, and registration
- Up-to-date immunizations: TB, MMR, Varicella, HepB, TDap (within 10 years), Flu
- Ability to see COVID+ and COVID PUI patients when needed
Responsibilities:
- Deliver comprehensive physical therapy services to diverse patient populations
- Complete timely and accurate documentation for each visit
- Service multiple communities within the assigned territory, ensuring coverage as needed
- Maintain productivity expectations through various visit types, including OASIS assessments and discharges
