Schedule/Hours:

  • Availability may include weekends and flexible scheduling to meet patient needs
  • 36 hours per week with a point-based productivity system for guaranteed assignments
  • Occasional travel beyond core territory may be required

Qualifications:

  • Current California Physical Therapist (PT) license
  • Basic Life Support (BLS) certification
  • Valid drivers license, reliable transportation, active auto insurance, and vehicle registration
  • Current TB screening (annual), MMR, Varicella, HepB, Tdap (within 10 years), Flu vaccine (annual), and drug screening
  • Physical exam clearance
  • Willingness to see a diverse patient base, including COVID+ and PUI patients

Key Responsibilities:

  • Deliver hospital-based physical therapy care in home and community settings
  • Complete comprehensive patient evaluations and develop individualized care plans
  • Conduct OASIS documentation and manage patient caseload efficiently
  • Ensure timely communication with patients, care teams, and physicians
  • Maintain accurate patient records and comply with all regulatory guidelines
