Schedule/Hours:

  • Primarily weekdays with the potential for weekend coverage.
  • Minimum weekly expectations and travel flexibility may be required.

Qualifications:

  • Active California Physical Therapist license.
  • Current BLS certification.
  • Valid drivers license, auto insurance, and registration.
  • Recent TB screening; immunizations: MMR, Varicella, HepB, TDap (within 10 years), annual flu.
  • Ability to pass a drug screen and physical.
  • Reliable vehicle for local travel.
  • Previous hospital or home health therapy experience preferred.

Responsibilities:

  • Deliver high-quality physical therapyconducting evaluations, routine visits, discharges, and follow-up care across a diverse territory.
  • Complete all documentation and coordinate care with physicians and interdisciplinary teams.
  • Ensure care according to best practice guidelines, including OASIS assessments and recertifications.
  • Flexibly respond to staffing needs, including occasional travel outside assigned areas and weekend shifts.
  • Provide services to various patient populations, including Covid+ and PUI patients when needed.
