Schedule/Hours:
- Primarily weekdays with the potential for weekend coverage.
- Minimum weekly expectations and travel flexibility may be required.
Qualifications:
- Active California Physical Therapist license.
- Current BLS certification.
- Valid drivers license, auto insurance, and registration.
- Recent TB screening; immunizations: MMR, Varicella, HepB, TDap (within 10 years), annual flu.
- Ability to pass a drug screen and physical.
- Reliable vehicle for local travel.
- Previous hospital or home health therapy experience preferred.
Responsibilities:
- Deliver high-quality physical therapyconducting evaluations, routine visits, discharges, and follow-up care across a diverse territory.
- Complete all documentation and coordinate care with physicians and interdisciplinary teams.
- Ensure care according to best practice guidelines, including OASIS assessments and recertifications.
- Flexibly respond to staffing needs, including occasional travel outside assigned areas and weekend shifts.
- Provide services to various patient populations, including Covid+ and PUI patients when needed.
