Qualifications:
- At least 2 years of recent, full-time occupational therapy experience in a hospital or home health setting
- Valid OT license in California
- Current Basic Life Support (BLS) certification
- Strong assessment and documentation abilities
- Excellent communication skills and adaptability in dynamic environments
Primary Responsibilities:
- Perform comprehensive home health evaluations and develop individualized care plans
- Collaborate closely with physicians, nurses, and other therapists to coordinate holistic care
- Deliver evidence-based therapeutic interventions in patients homes, focusing on restoring mobility, independence, and daily function
- Educate patients and caregivers on best practices, safety, and the use of adaptive equipment
- Maintain thorough, accurate, and timely clinical documentation
- Monitor patient progress, adjusting therapy plans as needed to maximize outcomes
