Primary Responsibilities:

  • Perform comprehensive home health evaluations and develop individualized care plans
  • Collaborate closely with physicians, nurses, and other therapists to coordinate holistic care
  • Deliver evidence-based therapeutic interventions in patients homes, focusing on restoring mobility, independence, and daily function
  • Educate patients and caregivers on best practices, safety, and the use of adaptive equipment
  • Maintain thorough, accurate, and timely clinical documentation
  • Monitor patient progress, adjusting therapy plans as needed to maximize outcomes
