Job Description

Deliver comprehensive physical therapy services within a supportive hospital environment serving multiple counties. This contract opportunity offers meaningful work evaluating, treating, and supporting patients as they progress toward greater independence and improved quality of life. Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • No on-call responsibilities

Key Qualifications:

  • Current Physical Therapist license in Georgia, or valid compact license
  • Minimum 2 years experience in home health and proficiency with HomeCare Homebase strongly preferred
  • Experience using various evaluation and assessment tools for comprehensive patient care
  • Reliable transportation to cover multiple counties; mileage reimbursement provided

Primary Responsibilities:

  • Conduct detailed physical therapy evaluations and develop individualized treatment plans based on physician orders
  • Monitor patient progress, reassess treatment goals, and modify care as necessary
  • Maintain meticulous records, including evaluations, treatments, and billing, in compliance with organizational, state, and federal requirements
  • Communicate effectively with patients, their families, and medical teams regarding goals, progress, and discharge planning
  • Recommend and facilitate adaptive equipment as needed to maximize rehabilitation outcomes
  • Provide supervision to Physical Therapist Assistants and Aides, ensuring quality and compliance in all care activities
  • Participate in professional development, internal training programs, and maintain required continuing education
