Schedule/Hours
- 36 hours per week
- MondayFriday shifts
- Alternate weekend rotation
Qualifications
- Active Occupational Therapy (OT) license
- Skilled Nursing experience preferred, especially in a hospital/rehab setting
- Previous experience with patients in post-acute or long-term care settings valued
- Excellent communication, collaboration, and adaptability skills
Responsibilities
- Conduct patient assessments and develop comprehensive treatment plans tailored to individual needs
- Collaborate with an interdisciplinary team to support patient-centered outcomes
- Document progress, maintain accurate records, and adjust interventions as clinically indicated
- Educate patients and families on progress and ongoing care requirements
