Schedule/Hours:

• Multiple scheduling options available: guaranteed or non-guaranteed hours.
• Weekend coverage may be required to secure a full case load.
• Flexible territory assignments within Alameda and Contra Costa Counties.
• Must be open to seeing Covid+ and Covid PUI patients.

Responsibilities:

• Deliver evidence-based occupational therapy services in a home health setting.
• Complete OASIS and routine evaluations, re-certifications, follow-ups, and discharges.
• Travel throughout assigned territory for patient visits, providing care and support.
• Accurately document visits and collaborate closely with physicians and interdisciplinary team members.
• Support patients in regaining independence and improving quality of life in their own homes.

Qualifications:

• Current and valid CA Occupational Therapist license.
• BLS certification (American Heart Association).
• Valid drivers license, auto insurance, and auto registration.
• Up-to-date immunizations: TB (annual), MMR, Varicella, Hep B, TDap (within 10 years), and annual flu vaccine.
• Physical and drug screen clearance.
• Home health experience preferred but not requirednew graduates are encouraged to apply.
