Schedule/Hours:
- Travel assignment, full-time hours
Qualifications:
- Graduate from an accredited Occupational Therapy program
- Active Occupational Therapist license in New York State
- At least one year of experience in Long Term Care or Home Care settings
- Recent Home Care experience (within the last year) preferred
- Proficient with OASIS documentation
- Familiarity with community resources and OT equipment operation/maintenance
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
Key Responsibilities:
- Develop and revise individualized treatment and discharge plans
- Provide direct skilled therapy and supervise COTAs
- Consult and educate patients, families, and care teams
- Coordinate care with interdisciplinary professionals
- Uphold privacy, dignity, and confidentiality standards
- Maintain regulatory compliance with state and JCAHO/OSHA guidelines
- Participate in quality improvement, professional development, and performance tracking
