Desired Experience and Qualifications

  • Graduate of an accredited Occupational Therapy program
  • Current OT license to practice in New York State
  • Minimum one year of Long Term Care/Home Care experience (recent home care experience preferred)
  • Proficiency with therapy equipment, community resources, and OASIS documentation
  • Strong communication, organizational, and independent problem-solving skills
  • Collaborative spirit and commitment to quality patient care

Key Responsibilities

  • Develop and revise personalized treatment and discharge plans collaboratively with families, patients, physicians, and care teams
  • Initiate preventive and rehabilitative strategies, ensuring patient safety and satisfaction
  • Educate patients, families, and team members on disease management and self-care
  • Supervise COTAs and Home Health Aides to uphold care quality
  • Maintain and ensure compliance with all clinical documentation and regulatory standards
  • Support continuous quality improvement in service delivery and team culture
  • Participate in care conferences, training, and the advancement of new initiatives
