Schedule/Hours:

• Schedule determined by patient caseload and geographic region within PHHs service area

Desired Qualifications:

• Current Occupational Therapist licensure, registered in Massachusetts
• Minimum 1 year of home care OT experience
• At least 2 years of experience in acute care, rehab, or homecare settings
• Proficient with various computer software applications; experience with hand-held patient care technology preferred
• Strong customer service skills and ability to relate to individuals from diverse backgrounds
• Excellent verbal, written, and electronic communication abilities
• Ability to prioritize, multitask, and adapt in a rapidly changing environment
• Willingness to travel within the assigned geographic area

Key Responsibilities:
