Provide expert occupational therapy care directly in patients homes as a Home Health Occupational Therapist (OT) focusing on enhancing daily living activities and independence. This role emphasizes comprehensive patient evaluations, individualized care planning, and collaboration with an interdisciplinary team to ensure optimal patient outcomes in a supportive field-based setting.

Key Responsibilities:

• Perform thorough occupational therapy evaluations and OASIS assessments as required.
• Develop and implement individualized treatment plans targeting ADLs, IADLs, cognition, upper extremity function, and home safety.
• Deliver skilled interventions including therapeutic activities, energy conservation, adaptive equipment training, and environmental modifications.
• Recommend appropriate durable medical equipment (DME) based on patient needs.
• Monitor patient progress and update treatment plans accordingly.
• Educate patients and caregivers on functional independence, fall prevention, and safety strategies.
• Maintain accurate, timely documentation using Home Care Home Base (HCHB) electronic system.
• Collaborate with nurses, PTs, SLPs, social workers, and home health aides to coordinate comprehensive care.
• Participate in case conferences, interdisciplinary meetings, and pursue ongoing professional development.
• Comply with agency policies, regulatory standards, and Medicare home health guidelines.

Qualifications:

• Active Occupational Therapist license in good standing within the practice state.
• Minimum 1 year recent Home Health OT experience; field experience preferred.
• Experience with OASIS assessments is required.
• Proficiency with Home Care Home Base (HCHB) preferred.
• Strong clinical assessment, functional training, and communication skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Ability to work independently with excellent organizational and time management skills.

Job Details:

• Location: Monument, CO a scenic community with easy access to outdoor recreational activities and a strong sense of community.
• Type: Full-time Contract (13 weeks)
• Schedule: Monday Friday, 8:00 AM 5:00 PM
• Caseload: Approximately 2532 points weekly (57 patient visits/day)
• Mileage reimbursement provided for daily travel within assigned territory
