Job Description

A licensed Occupational Therapist is needed to deliver skilled, in-home occupational therapy services to patients in the Grand Junction, CO area. This contract role focuses on providing patient-centered, evidence-based care in accordance with physician orders, payer requirements, and regulatory standards. The therapist will work independently in patients homes to enhance functional independence, safety, and quality of life through customized treatment plans.

Responsibilities:

  • Conduct evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits
  • Develop and implement individualized occupational therapy plans based on patient needs and physician orders
  • Provide skilled interventions, including ADL/IADL training, upper extremity strengthening, fine motor coordination, adaptive equipment training, energy conservation, and fall-prevention strategies
  • Educate patients and caregivers on treatment, adaptive techniques, home programs, and safety
  • Communicate patient progress and changes to the interdisciplinary care team
  • Oversee Occupational Therapy Assistants (OTAs) when applicable and allowed under Colorado law and agency policy
  • Complete accurate, timely documentation in Homecare Homebase (HCHB) adhering to Medicare Conditions of Participation and privacy regulations
  • Collaborate effectively with clinical staff including RNs, PTs, STs, and leadership

Qualifications:

  • Current, unrestricted Colorado Occupational Therapist license
  • Graduation from an accredited Occupational Therapy program
  • Valid drivers license and reliable transportation for home visits
  • Ability to practice independently in a home-based clinical environment
  • Proficiency or willingness to learn Homecare Homebase (HCHB) Preferred:
  • Experience in home health settings
  • Understanding of Medicare documentation and home health regulations
  • Experience with geriatric and medically complex patients
