As an Occupational Therapist in this dynamic environment, you will work predominantly in home health and community settings, serving counties within Fremont and your choice of one neighboring area. This is an exceptional opportunity for both new grads and experienced OTs to expand their resume with exposure to diverse patient populations, a variety of evaluation and treatment interventions, and modern approaches to care coordination. Schedule/Hours:
- Flexible schedule with guaranteed and non-guaranteed hours options available
- Occasional weekend coverage may be required
- Assignments across multiple nearby county regions
Key Qualifications:
- Current CA Occupational Therapist license required
- BLS certification
- Valid drivers license, auto registration and insurance
- Up-to-date immunizations (TB, MMR, Varicella, HepB, TDap, Flu)
- Willingness to travel within and potentially outside your assigned service area
- New graduates welcome; home health experience preferred but not required
Core Responsibilities:
- Conduct OASIS assessments, routine visits, evaluations, and discharge planning
- Provide comprehensive OT interventions to patients in their homes
- Document patient progress and communicate with interdisciplinary teams
- Serve a diverse caseload including post-acute and rehabilitative patients
- Maintain productivity standards as outlined in assignment guidelines
