Job Description
A home health occupational therapy role is available in the Charlotte, NC area, focusing on providing skilled therapy visits to patients recovering from orthopedic surgeries, managing chronic illnesses, and supporting those with functional limitations. This contract position offers autonomy in the field, with a schedule averaging six patient visits per day and mileage reimbursement for travel.
Responsibilities:
- Conduct occupational therapy visits focusing on ADL and IADL training, endurance building, and adaptive equipment education.
- Develop individualized care plans aimed at maximizing patient independence and preventing hospital readmissions.
- Collaborate effectively with the clinical team, maintaining clear communication.
- Use electronic systems such as EPIC Rover (view only), Focura, and Homecare Homebase to document and access patient information.
- Travel within Northern Charlotte and surrounding areas including Huntersville, Cornelius, Davidson, Morrisville, and Salisbury, with no maximum mileage limit.
Qualifications:
- Current NC State OT License and BLS certification.
- Valid drivers license and proof of insurance.
- Minimum of 1 year experience in home health occupational therapy.
- Strong skills in patient assessment, care planning, and education.
- Experience with Homecare Homebase (HCHB) required.
- Ability to recognize signs and symptoms to prevent patient readmission.
Schedule & Work Environment:
- Approximately 6 visits per day.
- No weekend rotation, call, or major holiday coverage required.
- Schedule cycles every 13 weeks with manager approval for return to office.
- Department culture values teamwork and supportive leadership.
- Orientation includes a 5-day new employee onboarding experience.
- Standard grey scrubs and easy parking available.
