Schedule/Hours:
- Guaranteed hours option: 36 hours/week min., productivity expectation of at least 27 points per week (30 points required for guarantee eligibility)
- Non-guaranteed hours: Flexible caseload, weekend coverage as needed
Qualifications:
- Current CA Occupational Therapy license (OT)
- BLS certification
- Valid drivers license, auto insurance, and registration
- Up-to-date TB, MMR, Varicella, HepB, TDap, Flu
- Drug screen and physical exam
- Reliable transportation for daily travel
Key Responsibilities:
- Manage a varied home health caseload including OASIS and non-OASIS visits
- Complete thorough documentation, evaluations, and care planning
- Collaborate with patients, caregivers, and interdisciplinary teams
- Travel between assigned territories, providing personalized therapy
