Job Description

Are you an LPN looking to shine in the home health arena? Look no further! We're seeking passionate LPNs to join our team in Nashville, TN, for an exhilarating 13-week contract.

Responsibilities:

Perform home visits and visits to assisted living facilities, ensuring exceptional patient care.
Meet weekly productivity targets of 25 to 35 units.
Participate in the on-call rotation every 4th week, providing support when needed.
Utilize electronic medical records systems, including Homecare/Homebase EMR, OASIS, and Outlook, for efficient documentation.

Requirements:

Active LPN license in Tennessee.
Current BLS certification (American Heart Association or American Red Cross).
Prior experience in home health care.
