Desired Qualifications:

  • Valid Colorado Special Services Provider (SSW) license required
  • Previous experience as a Social Worker in K-12 school settings preferred
  • Strong understanding of 504 Plan case management, including conducting meetings and evaluations
  • Demonstrated ability to provide mental health counseling to children and adolescents
  • Proficiency with suicide and risk/threat assessments and crisis intervention
  • Excellent communication, organizational, and record-keeping skills

Primary Responsibilities:

  • Manage and monitor 504 Plans, ensuring accurate evaluations and timely updates
  • Facilitate and conduct 504 Plan meetings with staff and families
  • Deliver mental health counseling services to students on the 504 caseload and the general education/at-risk population
  • Participate in and support crisis intervention, suicide, and risk/threat assessments as needed
  • Collaborate with teachers, school psychologists, and families to create holistic support networks for students
  • Document services, interventions, and recommendations per district and state guidelines
