A healthcare professional with experience in school nursing and case management is needed for a contract position in Solvang, CA. This role requires a Licensed Vocational Nurse (LVN) to provide care and coordination for a student with medical needs, supporting the completion of a triennial reassessment.
Responsibilities include:
- Conducting a thorough triennial reassessment for a student with medical requirements
- Collaborating with parents and the student in person at least once to understand and address individual concerns
- Developing and updating health care plans tailored to the student's needs
- Coordinating support services related to the students medical aide
- Ensuring compliance with school and healthcare protocols
Qualifications:
- Valid LVN license and relevant clinical experience
- Experience in school nursing or pediatric case management, preferably within educational settings
- Strong communication and interpersonal skills to effectively engage with families
- Ability to work autonomously and coordinate between healthcare providers and school staff
- Availability to work approximately 3 hours per week, with flexibility to spread hours throughout the week or complete in a single day
This role is fully in person, requiring presence at the school and interactions with the student and family. The position is set to start ASAP, with a targeted start date of June 3rd, 2026.
