Job Description

A healthcare professional with RN Case Management experience is needed to provide skilled nursing services at a school in Santa Barbara, CA. This contract position requires licensed vocational nurse (LVN) availability for approximately 3 hours per week. The schedule is flexible, allowing hours to be spread throughout the week or concentrated into one day.

Key qualifications and experience:

  • Licensed Vocational Nurse (LVN) certification
  • Experience in school nursing and case management
  • Ability to conduct comprehensive triennial reassessments, including updating health care plans
  • Familiarity with coordinating medical aide support for students with medical needs
  • Strong communication skills to effectively meet and collaborate in person with the student and their parents

Primary responsibilities include conducting a thorough triennial reassessment for a child with ongoing medical needs. This involves reviewing and updating health care plans, coordinating with medical aides, and addressing all health concerns collaboratively. An in-person meeting with both the student and parents is essential to ensure the nursing plan fully supports the student's needs.
