Qualifications:

• RN licensure in the state of California (active and in good standing)
• Prior experience in school settings and case management preferred
• Ability to coordinate triennial reassessments and develop individualized healthcare plans
• Strong interpersonal skills for effective communication with students, families, and school staff

Responsibilities:

• Conduct a triennial reassessment for a child with medical needs
• Develop and update health care plans and coordinate with the childs medical aide
• Meet with the student and parents at least once in person to address concerns and support care
• Provide professional guidance and clear documentation throughout the assignment
