Job Description
A Licensed Vocational Nurse (LVN) role is available on a contract basis in Carpinteria, CA, supporting a school setting with specialized nursing services. This position requires approximately three hours per week, offering scheduling flexibility that allows the hours to be distributed throughout the week or completed in a single day. The start date is ASAP, with an expected commencement by June 3rd, 2026.
This role focuses on conducting a triennial reassessment for a child with medical needs. Responsibilities include developing and updating health care plans and coordinating medical aide services. The nurse will work collaboratively to support and coordinate care, ensuring a comprehensive approach to the student's health requirements.
Key Responsibilities:
- Conduct thorough triennial health reassessments for students with medical needs
- Develop, review, and update individualized health care plans
- Coordinate and support medical aide services within the school setting
- Engage with the student and parents through at least one in-person meeting to understand and address specific health concerns and needs
- Collaborate with school staff and healthcare providers to ensure seamless care delivery
Desired Qualifications:
- Current LVN licensure in the state of California
- Experience in school nursing or pediatric care, preferably with case management skills
- Ability to conduct detailed health assessments and develop care plans
- Strong communication skills for effective collaboration with families and multidisciplinary teams
- Flexibility to manage scheduling based on school needs
