Qualifications & Requirements:
- Active California Physical Therapy (PT) license required
- Credentialed or eligible for school-based employment in California
- Experience in school or transitional programs supporting young adults (ages 18-22) is preferred
- Strong collaboration, communication, and documentation skills
- A passion for empowering students with diverse abilities
Key Responsibilities:
- Provide physical therapy assessments and deliver direct services to students in the 18-22 youth program
- Develop and implement individualized treatment plans based on student needs, IEP goals, and transitional objectives
- Consult and collaborate with special education staff, families, and service agencies
- Maintain organized records and contribute to progress reporting and IEP meetings as needed
- Support safe and inclusive participation in daily and vocational activities
