Title: RN Hospice Travel Nurse - 13 Week Contract

Location: Valparaiso, IN

Type: Contract, Full-time (40 hours per week)

Duration: 13 weeks

Experience: Experience with Homecare Home Base preferred

Job Description:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our hospice team for a 13-week travel contract. As an RN Hospice Travel Nurse, you will provide exceptional end-of-life care to patients in their homes, ensuring comfort and dignity during their final journey.

Responsibilities:

Provide skilled nursing care to hospice patients in their homes
Conduct thorough assessments of patients' physical, emotional, and spiritual needs
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Administer medications and treatments as prescribed
Educate patients and their families about disease progression, symptom management, and end-of-life care
Provide emotional support and guidance to patients and their loved ones
Coordinate care with other healthcare professionals and community resources
Document patient care activities accurately and timely using electronic medical records (EMR), preferably Homecare Home Base

Qualifications:

Active Registered Nurse (RN) license in Indiana (IN)
Minimum of 1 year of recent hospice or home health experience
Experience with Homecare Home Base preferred
BLS certification
Strong communication and interpersonal skills
Compassionate and empathetic demeanor
Ability to work independently and as part of a team
Reliable transportation
