Job Description

Description: We are hiring a compassionate Home Health Registered Nurse for a 13-week contract opportunity in Chestertown, MD. If you have a passion for delivering quality care to patients in their homes, we want to hear from you. This role requires proficiency in the HomeCare/HomeBase EMR system and previous experience in home health. Full-time hours are available, Monday through Friday, with flexible scheduling.

Responsibilities:

Conduct thorough assessments and develop personalized care plans for each patient.
Administer medications, treatments, and therapeutic interventions as prescribed.
Collaborate with interdisciplinary teams to coordinate patient care and services.
Educate patients and families on disease management strategies and healthy lifestyle choices.
Maintain accurate and detailed documentation of patient care activities.

Requirements:

Active RN license in the state of Maryland.
Current BLS certification from the American Heart Association or the American Red Cross.
Minimum of one year of experience in home health nursing.
Proficiency in using the HomeCare/HomeBase EMR system.
Strong interpersonal skills and the ability to work effectively in a multidisciplinary team environment.
