Responsibilities:

Collaborate closely with our interdisciplinary team to develop personalized home care plans, tailored to meet the unique needs of each patient.
Monitor and accurately document patient conditions and progress, ensuring meticulous attention to detail.
Administer medications and deliver essential wound care with compassion and skill.
Utilize HomeCare HomeBase for streamlined documentation and effective communication.

Requirements:

Active RN licensure in Connecticut.
Minimum one year of recent home health experience.
Proficiency in HomeCare HomeBase.
Previous experience with home-to-home workload is valued.
Hold a current BLS Certification from the American Heart Association.

Details:

Full-time positions, offering 40 hours per week.
Shifts scheduled from 8:00 AM to 5:00 PM.
Mileage reimbursement available for travel within Stamford.
