Position: RN Supervisor (PRN)

Location: Oak Park, Illinois

Schedule: Approximately 8 hours/week, 8:00 AM - 4:30 PM, Monday to Friday

Job Summary: Join our team at Cook County Health and Housing Forward’s Medical Respite Center program, located in Oak Park, Illinois. We are seeking a dedicated RN Supervisor to provide clinical oversight and support in our 19-bed floor dedicated to serving individuals experiencing homelessness. This role involves coordinating post-acute-care medical respite services, including outpatient antibiotic therapy, wound care, home hospice care, and isolation/quarantine for CoVID-19.

General Responsibilities:

  • Conduct clinical intake assessments for new clients entering the program.
  • Supervise medical assistants, providing guidance on care plans and ensuring delivery of high-quality care.
  • Coordinate client care with hospital providers, pharmacies, and primary/specialty care clinics.
  • Collaborate with volunteer physicians on client clinical concerns.
  • Assist clients in managing medical issues, including administering IV antibiotics and wound dressing changes.
  • Ensure accurate documentation of client progress in the data management system.
  • Maintain medical assistant work schedules to ensure 24/7 clinical coverage.
  • Arrange transportation for clinical appointments and coordinate telehealth visits.
  • Manage clinical and office supplies and provide written hand-offs to receiving agencies for discharged clients.
  • Perform other duties as assigned.

Minimal Qualifications:

  • Bachelor’s Degree or diploma in nursing.
  • Current state licensure as a registered nurse.
  • American Heart Association BLS certification.
  • Basic computer skills, including Microsoft Excel and Word.
  • Familiarity with basic infection control protocols and procedures.

Preferred Qualifications:

  • Familiarity with Spanish or Polish languages.
  • Experience in home health care.
  • Strong connections with public health or experience collaborating in public health initiatives.
  • Previous experience working with individuals experiencing homelessness.
  • Understanding of trauma-informed care philosophy and practices.

  • Demonstrated emotional strength and caring nature.
  • Ability to adapt in a fluid environment.
  • Comfort working with clients with diverse needs, including social and behavioral needs.
