Job Description

Seeking a dedicated Registered Nurse for a full-time position in Largo, Maryland. Join our home health team and make a difference in patients' lives. This role offers guaranteed 40 hours per week, Monday through Friday, from 8 am to 5 pm.

Shift:

Monday through Friday (8am to 5pm).
Full time position.
13 Week long contract.

Responsibilities:

Perform comprehensive assessments of patients' health status.
Develop and implement individualized care plans based on patient needs.
Provide skilled nursing care and medication management as prescribed.
Coordinate with interdisciplinary team members to ensure continuity of care.
Educate patients and their families on disease management and treatment plans.
Document patient care accurately and timely using the OASIS and HomeCare HomeBase EMR system.
Monitor and evaluate patient progress, adjusting care plans as necessary.
Collaborate with healthcare providers to optimize patient outcomes.

Requirements:

Active RN license in Maryland.
Current BLS certification from the American Heart Association or the American Red Cross.
Proficiency in OASIS and HomeCare HomeBase EMR system.
Strong communication and interpersonal skills.
Ability to work autonomously and as part of a team.
Compassionate and patient-centered approach to care.
